A cramped shoe or a change in running load can contribute to forefoot discomfort. But “metatarsalgia” describes a location of pain rather than a single diagnosis; joints, nerves and bones can all be involved.
Notice whether the pain is diffuse or sharply localised, whether there is swelling, and whether walking is becoming uncomfortable. While shoe fit and training changes may help some presentations, a focal point of bony pain after a training increase needs assessment rather than repeated trial runs.
WHEN TO GET HELP
Persistent focal tenderness, swelling or pain with normal walking raises concern for a bone stress injury.
